How to Submit a Proof of Claim:
Proof of Claim (“POC”) forms and the required form are available on this website under Proof of Claim and Information.
Completed Proofs of Claim should be submitted by email to:
Claims may also be mailed to the address listed on this website.
Fax submissions are not accepted.
Please do not submit the same Proof of Claim by multiple delivery methods.
General Requirements
- The Proof of Claim form must be completed in full and signed
- Notarization is not required
- Scanned signatures are acceptable
- Please include the Key Insurance claim number in the email subject line, if known
- If applicable, include:
- Key Insurance claim number and/or policy number (if known)
- Date of loss
- Name of your insured or client
- Any internal claim or file reference number you use for tracking
To assist with processing your Proof of Claim, we recommend that you do the following:
Claimant Information
The Claimant is the person or entity seeking payment.
Examples:
- Subrogation claims: the insurance company (e.g., ABC Insurance Company, as subrogee of John Doe)
- Vendor or service claims: the vendor
- Attorney-submitted claims: the claimant should reflect the party asserting the claim; payment direction may involve counsel
Undetermined Claim Amounts
Claims may be submitted with an undetermined amount, if possible please provide an explanation (e.g., pending salvage proceeds or unresolved damages).
Property Damage / Subrogation Claims
If available, please provide a complete subrogation demand, including:
- A demand letter on company letterhead
- A clear breakdown of the net amount claimed
- Supporting documentation relevant to the claim
- Your internal claim or file reference number
Injury-Related Claims
If available, please include:
- Incurred medical bill totals and other injury-related amounts claimed
- Payment ledgers
- Coverage types used (e.g., PIP, BI, MedPay, UM/UIM)
- Signed releases, if applicable
- Any summaries you would like reviewed
Please do not submit medical records. Medical records are not required and will not be reviewed.
Review Proces
- Proofs of Claim are logged as received
- Processing times vary
- Please update or amend your Proof of Claim if you have additional information, an amount becomes final, or your contact information (email, phone or address) changes.
- If additional information is required, we will contact you. Please provide an email to expedite communications.
Submission of a Proof of Claim does not constitute a determination of coverage, approval, or payment.
